  • The SF-36® Health Survey is a 36-item questionnaire used to assess patient-reported health-related quality-of-life outcomes. The SF-36® Health Survey measures eight domains of health: Physical functioning Role-Physical Bodily pain General health Vitality Social functioning Role-Emotional Mental health It yields an eight-scale profile of norm-based scores (one for each of the eight domains of health) as well as physical and mental health component summary scores, a self-reported health transition rating, a response consistency index (RCI), and a preference-based health utility index (SF-6D).
